This Moves/Adds/Changes (MAC) form is only for the FUSD (Fremont Unified School District). If you are an existing customer but are not a member of the FUSD, please use our regular MAC form.

To submit a FUSD MAC request, FUSD members should fill out this form as completely as possible (contact us if you have questions). Once the information is submitted, it will be sent to our headquarters for completion and further processing. We will contact you for additional information and to provide you with material and labor costs as necessary.
